    AMR’s Integrated Healthcare programs are designed to utilize the appropriate provider, tools and technologies required to best meet the various healthcare needs of patient populations. AMR is seeking to hire a small team of experienced nurses to lead a network of dedicated  nurse navigators who will remotely triage patients and provide unpresented care coordination and patient navigation services to a unique population across the D.C. area. As an integral part of AMR’s Medical Command Center your efforts will be supported by innovative communications and logistical management platforms that are specifically designed to improve large patient population’s access to care, experience, and clinical outcomes. This position presents the opportunity to be a part of a new, exciting and innovative healthcare delivery model in the pre-hospital setting.

     

    JOB TITLE: Registered Nurse-Command Center

    REPORTS TO: Clinical Nurse Manager

    FLSA CLASS: Non-Exempt/Hourly

     

    SUMMARY:

    This role has Primary responsibility to link mobile resources together, integrate and coordinate them to respond appropriately to patient needs. Serve as a dynamic leader in communication center to link 911 services, Nurse Navigators and mobile clinical care teams utilizing innovative communications and logistical management platforms. This role will also be responsible for participating in advanced medical research and treatment modalities as directed by the Medical Director and AMR Clinical leadership. This position will interact with area health care, social service and transportation providers to assist with medical advice, appointment scheduling and required follow-up contacts to assure the highest quality of patient care.

     

    ESSENTIAL FUNCTIONS:

    • Serves as Communications Center leader and liaison with healthcare partners.
    • Works with AMR’s Integrated Healthcare Program and partners to develop new avenues of patient pathways to access healthcare
    • Creates and maintains a positive and cooperative working environment in a virtual setting
    • Participates in activities that promote AMR Integrated Healthcare Program and AMR organization at large
    • Participates in quality assurance review of patient Inbound/Outbound calls and patient satisfaction surveys.
    • Communicates with physicians and other healthcare agencies

     

    PRIMARY DUTIES:

    • Shift leader in virtual setting providing oversight to Nurse Navigators and Non Clinical Call Takers
    • Triages and assess patients, coordinating care needs across health care continuum
    • Coordination of Occupational Medicine services for employees injured at work
    • Surveillance and monitoring Medical Clearance forms for new employees
    • Oversight telemonitoring services in variety of settings
    • Provides instruction to patients regarding health and treatment plans
    • Lead and facilitate Integrated Healthcare training course provided by AMR
    • Conducts patient follow-up satisfaction
    • Prepares and reviews documentation, agency reports and statistical data
    • Participates in training, staff development, and quality assurance activities
    • Constantly demonstrates and maintains a high standard of confidentiality
    • Assist in community resource development
    • Assist in the development of Integrated Health Procedures/Flow maps
    • Ensures resource list is up to date and has a specific flow process with each
    • Assist in development of new protocols
    • Understands challenges and demands of Healthcare reform
    • Provides excellent Customer Service
    • Perform other duties as assigned by AMR Clinical Leadership

     

    WORKING CONDITIONS:

    • Works alone, with and around other, face to face and verbal contact

     

    PHYSICAL DEMANDS:

    • Walking outside, lifting no greater than 10 pounds, pushing, pulling, reaching, typing, balancing
    • Frequently hearing/listening, clear speech, touching, smelling, walking indoors and outdoors, sitting and constantly observing.

     

    MINIMUM REQUIREMENTS:

    • Graduate of an accredited school of nursing
    • >5 years of experience as a RN in Emergency settings
    • Experience with all patient ages, from Pediatric to Geriatric
    • Current CPR card;
    • Maintain active state RN license with ability to obtain multi state licensure
    • Strong and effective verbal, written, and interpersonal communication skills;
    • Strong personal management and staff development skills
    • Excellent time management and organizational skills
    • Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills;
    • Ability to adhere to established standards for educational quality;
    • Familiar with basic computer applications and functions.

     

    PREFERRED REQUIREMENTS:

    • Demonstrated teaching and educational facilitation skills;
    • Proficient Bilingual translation skills
    • Experience with clinical data gathering and Ethical Principles;
    • Master’s or Bachelor’s degree in Healthcare Administration, Public Health or any other degree with a focus on community or disease education;
    • Have an understanding of quality assurance and improvement processes
